Diagnosis and surgical treatment of an epidermoid cyst at the level of the coronary band in a 9-year-old horse

Objectives:
Epidermoid cysts are considered to be non-neoplastic mass lesions. In the horse they have been reported at multiple locations such as cranium, gastro-intestinal tract, skin, testis and the distal limb. This case report describes an epidermoid cyst at the level of the coronary band, appearance on CT, histopathologic findings and surgical intervention including 5 months follow-up.
Methods:
A 9-year-old Warmblood gelding was referred to the Equine Hospital of Utrecht University for CT examination and surgical biopsy of a slowly growing mass located dorsoproximal to the coronary band of the right front foot. The mass had been slowly growing over the past 2,5 years and the horse showed 3/5 lameness since 1 month. Two months after the CT and treatment by the referring vet the horse returned to the Equine Hospital due to increased lameness, swelling of the leg and purulent discharge from the mass.
Results:
A second CT examination was performed. Surgical treatment consisted of total mass removal under general anaesthesia using an inverted T incision. Histopathological results of the complete mass were consistent with an epidermoid cyst. The wound healed well after a 1 month period of box rest and bandages. The horse is not lame 5 months after surgery and is gradually increasing training.
Conclusions:
CT was a useful imaging modality in this case and surgical intervention led to a successful outcome. Histopathology performed on local biopsies were inclusive and a definite diagnosis could be made based on total mass removal.
