Biomechanical comparison of three bone anchors implanted into cadaveric feline femurs

Objectives:

Traumatic ligament injuries are common in cats. They are usually treated by ligamentoplasty with a synthetic prosthesis that can be secured using bone anchors (BA). Few studies have evaluated the biomechanical strength of BA in cats. The objective is to compare the mechanical properties of three BA of a suitable size for use in the cat.

Methods:

Nine pairs of femora were harvested from adult cats (approved by an Ethics Committee) and randomly assigned to 3 anchor groups; a screw-type (STBA), an impacted (IBA) and an all-suture bone anchor (ASBA). All anchors were implanted at the femoral origin of the lateral collateral ligament. A single-cycle load-to-failure test (0.5 mm/s) at 90° to the axis of insertion after preconditioning (5-10 N, 10 cycles) was performed. Stiffness, load to failure, load at 3 mm of displacement, total elongation, mode of failure and ease of insertion were recorded.

Results:

STBA was the easier anchor to insert subjectively. IBA had a significantly lower load at 3 mm of displacement (55.06 N) and stiffness (21.43 N/mm) compared to STBA (90.36 N, 35.45 N/mm) and ASBA (86.92 N, 35.67 N/mm) (p<0.05). STBA had a significantly greater load to failure (292.90 N) than IBA (129.55 N) and no difference with ASBA (213.2 N). Several modes of failure were reported (suture or eyelet breakage and anchor pull-out).

Conclusions:

The mechanical properties of STBA and ASBA were superior to the IBA but all are expected to withstand physiologic loads during feline walk, trot and jump. Clinical evaluation is mandatory before widespread use.
