Muscle damage and pain after midline, flank and laparoscopic ovariectomy in the bitch

Objectives:
Muscle damage and pain are part of the surgical trauma in the postoperative period. Creatine phosphokinase (CK) is a sensitive biomarker of muscle stress, considering its elevation proportional to the amount of muscle tissue involved. The aim of this study was to evaluate the degree of muscle damage in the postoperative period by determining serum and salivary CK levels after three different ovariectomy (OVE) techniques in the bitch, and its correlation with pain.
Methods:
Forty-five bitches were divided into 3 groups of fifteen animals each: group M (midline OVE), group F (flank OVE) and group L (laparoscopic OVE). Serum samples were collected before surgery and at 1, 24, 72 and 168 hours post-surgery; saliva samples were collected pre-surgery and at 24, 72 and 168 hours post-surgery. The short form of the Glasgow Composite Measure Pain Scale (CMPS-SF) was performed before surgery and 1, 4, 8,12 and 24 h post-surgery.
Results:
Serum CK significantly increased at 24 hours in group F, and levels were higher than those in group M and L. Salivary CK did not significantly rise in any group in the postoperative period. CMPS-SF scores in group F were higher than those in group L at 8,12 and 24h; in group M they remained higher for longer than in group L. Pain scores did not correlate with serum CK levels at any study time.
Conclusions:
Selecting the OVE technique that minimizes muscle damage and pain is crucial for patients wellbeing. Laparoscopic OVE should be preferred over flank and midline OVE when possible.
